Semi-Annual Progress Report Guidelines

On this page you will find guidance for the completion of the Semi-Annual Progress Report . Your progress report must describe both programmatic and fiscal activities. The first semi-annual progress report is due no later than 5:00 p.m. on March 30 of the funded year and the second semi-annual progress report is due no later than 5:00 p.m. October 30 of the funded year. The document should be submitted to Jan Brite, Bin #60 Arizona Department of Education, 1535 W. Jefferson Phoenix, AZ 85007.

The Match Verification Form must accompany your Semi Annual Report. It is essential that you show that you met your 50% of the total program cost (dollar for dollar) match on the Match Verification form.

Please Note: Required reports and deadlines are based on the requirements and deadlines of the Corporation for National Service, the funding source for the Learn and Serve Grant. If requirements change or additional information is required by the Corporation for National Service, it will be necessary to adjust state reporting accordingly. It is crucial that you meet the requirements and deadlines in order for the Arizona Department of Education to meet the deadlines set by the Corporation for National Service.

Semi-Annual Progress Report Outline

Section I

  1. Grantee Organization Name
  2. Grant Number
  3. Name, title, address and phone number of the individual who completed this report
  4. Total number of students/youth engaged in service-learning
  5. Total number of adult volunteers

Section II
Please address the following:

  1. Summary of Progress
    Please describe your project’s progress during this reporting period toward accomplishing annual objectives.
    • Describe progress toward the goals and objectives outlined in your Learn and Serve Arizona proposal.
    • Describe any research, program evaluation information or other indicators of progress to date.
  2. Primary Accomplishments
    escribe all accomplishments such as conferences, in-service activities, project milestones, significant community, participant or institutional impact, or unique successes.
    This section should also include:
    • Quotes from participants, parents, teaches community members and/or service recipients
    • News clippings
    • Information on public awareness activities (e.g. copies of press releases)
    • Descriptions of any awards or special recognition your program has received
  3. Primary Challenges
    Please describe challenges encountered by your project. Include issues/problems (both resolved and unresolved,) obstacles to achieving program objectives, significant sources of delay, events or incidents that caused concern and any need of which the Corporation for National Service should be aware.
  4. Primary Training and Technical Assistance Needs
    Describe the training/technical assistance needs that have surfaced during this reporting period. Include needs for which you have adequate resources and information as well as those that you are unable to meet. Training/technical assistance needs may include requests for professional guidance in program related areas such as curriculum development, fundraising, evaluation, management or staff development.
Please Note: With regard to publications created by “grantees,” copies must be sent to the Corporation for National Service’s Office of Public Affairs, the Learn and Serve America program office, and the National Service Learning Clearinghouse. Also, grantees are responsible for assuring that an acknowledgement of Corporation for National Service support and disclaimer appears in any external report or publication based upon work supported by the Learn and Serve America Grant.
